Your .service file is point towards /usr/lib/headphones, but the package is installed to /opt/headphones/

It seems that the headphones maintainer has changed their way to draft new versions, prefering keeping a master branch stable with regular fixes without having to release a new version. It has not been confirmed by him but is only my opinion. I keep this package, but I also created a -git version based on master here: https://aur.archlinux.org/packages/headphones-git/

@ldclrcq: it consumes a lot of memory, compared to couchpotato for example (200MB for headphones), but it does not appear to leak anymore.

Okay, glad it's not just me. That's always a bit scary when it isn't duplicated. I've downgraded for now. If nothing comes up, I'll try to upgrade and debug later in the week when I get more free time.

Anyone else have a nasty memory leak after the update? I saw an old post on git about it and how to debug it, which I haven't done... But just stopping headphones from running stops the memory leak. I have 32GB and had soaked up 50% of that. I rebooted and the leak continued. I stopped headphones and the leak stopped. Anyone else?

@dominicm: Sorry, I totally forgot to answer… The headphones.tmpfile is exactly here to solve the problem you had: it is used by systemd-tmpfiles to create the headphones folder in /var/run at boot (or when doing systemd-tmpfiles --create). It is standard, you do not have to run it.
