Another patch, this time to only use integer operations for volume calculations. PA_VOLUME_NORM and 100% should cleanly map to each other. http://pastebin.com/Efn1qLWB

Just a heads up: I spent the last few days rewriting this in C++. The interface has changed slightly, but the majority remains the same. New features (card handling/profile management) added. Bug reports always welcome on github, of course.
