I had the same problem as dxxvi. It looks like it's caused by the value of StartupWMClass in the .desktop files. The behavior changed once I had entered unique values for StartupWMClass in each .desktop file, but that said - I don't really understand how it works or what the optimal configuration is.

Since I upgraded to gnome 3.12, there is something wrong with this firefox icon/link. In the all applications section (click Activities, then click on the multi-dot square Show Applications at the bottom), there are 2 firefox icons: Firefox Beta and Firefox Beta - Safe Mode. I start the Firefox Beta application but the icon in the left panel (the one you see when clicking Activites) is Firefox Beta - Safe Mode. Does anybody know how to fix this? Thanks.

Firefox 29 just went stable, so it might be a good idea to update Beta.
